Depth of Focus of a new EDOF IOL with inverted meniscus design

Purpose: Evaluate visual performance at different distances, spectacle independence and photic phenomena of a binocular combination of two models of a new EDOF intraocular lens (IOL) with an inverted meniscus design, Art40 and Art70 (ArtIOLs, Voptica SL, Murcia, Spain). These IOLs improve peripheral optical quality and increase depth of focus.

Methods: Bilateral implantation was performed in 13 cataract patients with Art40 and Art70 models to induce low and high values ​​of spherical aberration, respectively.

One month after the last surgery, best corrected visual acuity (BCVA) and uncorrected visual acuity (UCVA) (LogMAR scale) were measured at infinity, 66 cm and 40 cm, using -1.5 and -2.5 D test lenses and SLOAN charts under photopic lighting conditions. Quality of Vision (QoV) and Patient-Reported Spectacle Independence Questionnaire (PRSIQ) were performed.

Results: The mean distance monocular BCVA was zero for Art40 and Art70. In eyes with Art40, the intermediate and near BCVA decreased to 0.14 and 0.41 LogMAR, respectively, while in eyes with Art70 those values ​​remained much higher, 0.04 and 0.19 LogMAR. The binocular combination of Art40 and Art70 improved BCVA to 0.01 for intermediate and 0.18 LogMAR for near vision. With the two models of inverted meniscus lenses, a target myopic refractive (-0.25 and -0.75 D for Art40 and Art70, respectively), Binocular UCVA were very similar to BCVA (0 LogMAR for infinity and 66 cm), however it increased to 0.04 LogMAR at 40 cm. None of the patients reported glare or halos and 92% of the patients never wore glasses for far and intermediate vision and 85 % for near vision.

Conclusions: The binocular combination of the two models of ArtIOLs (Art40 and Art70) significantly increased the depth of focus to at least 40 cm. This combination resulted in spectacle independence and great comfort at all distances without the compromise of halos or dysphotopsia. In addition, these new IOLs improve performance in the periphery thanks to the inverted meniscus shape.
